      What does methylcobalamin do that cyanocobalamin does not?

      It functions directly, without conversion. Mecobalamin is the cofactor for methionine synthase, the enzyme that remethylates homocysteine to methionine, and it donates its methyl group in that reaction before being recharged. Cyanocobalamin must first have its cyanide ligand removed and be converted into this or the adenosyl form before it can act.

      Why is mecobalamin used in nerve preparations?

      Because the methylation cycle it supports is central to myelin maintenance and to nerve function, and because it is already in the active form. It is widely used in neurological and peripheral neuropathy formulations, particularly in Japan and across Asia, where it has a long-established clinical position.

      How light-sensitive is mecobalamin?

      Considerably more than cyanocobalamin. The methyl ligand is readily cleaved by light, so the material and any product containing it must be protected from light throughout processing and in the finished pack. Amber or opaque packaging is standard, and exposure during manufacture should be minimised rather than assumed tolerable.

      Should both active forms be supplied together?

      They serve different enzymes in different cellular compartments, so some formulations include both. Mecobalamin works in the cytosol for methionine synthase; cobamamide works in the mitochondria for methylmalonyl-CoA mutase. Whether both are needed depends on the intended claim and the clinical rationale for the product.
