      What enzyme does cobamamide serve?

      Methylmalonyl-CoA mutase, a mitochondrial enzyme that converts methylmalonyl-CoA to succinyl-CoA. That step routes odd-chain fatty acids and several amino acids into the citric acid cycle for energy production, which is why cobamamide is associated with energy metabolism rather than with the methylation cycle that mecobalamin supports.

      Why is methylmalonic acid used as a B12 marker?

      Because it accumulates when this pathway is impaired. If cobamamide is deficient, methylmalonyl-CoA cannot be converted and methylmalonic acid rises in blood and urine. That makes it a more specific indicator of functional B12 deficiency than serum B12 itself, which can appear normal while tissue function is already compromised.

      Which is the predominant B12 form inside cells?

      Adenosylcobalamin, that is cobamamide, particularly in liver and muscle tissue where it is stored. Methylcobalamin predominates in plasma. That distribution reflects their different roles, with the mitochondrial form retained in tissue and the cytosolic form circulating.

      Does cobamamide need light protection?

      Yes, like mecobalamin. The 5-deoxyadenosyl ligand is light-sensitive and is cleaved on exposure, so the raw material and the finished product must be protected from light throughout handling, processing and storage. Amber or opaque packaging is standard for products containing it.
